The I Choose Wellness Passport tab gives you details on how to become an I Choose Wellness passport member. You can view the mechanics and the list of I Choose Wellness sites. You can also download the application form here. If you’re already a member, you can check your points with just one click to find out what awards you can claim. Just log in with your passport details and voilà!
