The "I Choose Wellness Program" is a wellness advocacy where members are provided extra motivation to get into a healthy lifestyle through rewards.

Who can Sign Up?

Membership is open to all NESTLÉ consumers, existing NESTLÉ CLUB members and individuals in Metro Manila who are at least 18 years of age at the time of joining and are committed to maintaining a healthy lifestyle. The passport will be issued to the first 32,000 people to sign up.

The full program mechanics posted at the booth should be read and understood prior to joining the program.

Who are not eligible?

Females who are pregnant at the time of joining are not eligible. Should you become pregnant after signing up, you won?t have to forfeit your membership but you will be disqualified from earning points during the term of your pregnancy.

Mothers with infants and young children up to 3 years of age are not allowed to participate in seminars , workshops, etc. that may deal with infant and young child feeding practices.

Employees of Nestlé Philippines, Inc. (NPI), One Incentive Systems Advocates, Inc. (1ISA), First Vision, Inc., their affiliates, subsidiaries, successors or assigns and their relatives up to the second civil degree of affinity or consanguinity and or members of their households (i.e. those sharing their home addresses) are disqualified from being Wellness Passport holders.

NPI reserves the right to recall the issuance of a Wellness Passport or remove from the roster any member at any stage of the Program on the ground of a member?s failure or refusal to abide by the terms and conditions of the Program or has been found guilty of fraud or misrepresentation. The decision of NPI on this regard shall be final.

How do I Sign Up?

  1. Proceed to the ?I Choose Wellness? booths located at participating supermarkets or at partner activity venues around Metro Manila.
  2. Read the TERMS & CONDITIONS posted at the booth.
  3. Secure from a booth attendant a Membership Form and fill out the same at the booth. Submit the Membership Form together with at least two valid IDs, with one of the IDs containing a photo of the member.
  4. Fill out the Lifestyle Questionnaire and submit the same to the booth attendant.
  5. Get your Wellness Passport and start earning points!

How do I earn and record points?

A member can earn points in 3 ways

  1. By Purchasing participating Nestlé branded products
    Every five hundred pesos (P500) worth of purchase on participating Nestlé branded products from any supermarket will entitle you to 25 points for regular passport holders and 35 Nestlé Club points for Nestlé Club passport holders.
    These points can only be credited on the month when the purchases were made and at authorized venue sites.
  2. By Participating in ?I Choose Wellness? Activities
    You can also earn points by attending the monthly I Choose Wellness Programs. Attendance and participation in any activity can earn sixty (60) points for regular passport holders and ninety five (95) Nestlé Club points for Nestlé Club member passport holders.
  3. By Undergoing Once a Month Free Nutrition Counseling
    Undergoing nutrition counseling will entitle you to one hundred fifteen (115) points per counseling for regular passport holders and one hundred forty (140) Nestlé Club points per counseling to Nestlé Club member passport holders.

However only one nutrition counseling per month will be awarded with points.

To record the points earned at the end of each activity, you must present your passport as well as your official receipts to the booth attendant who will give you your corresponding points for your little steps to wellness.

No passport, no points for the activity.

All points earned under this program are neither transferable nor convertible to cash.

Is there a maximum number of points one can earn?

A maximum of five thousand (P5,000) worth of purchases per month can earn points. One can only earn a maximum of 350 points per month on purchases of Nestlé products.

As for activities, a member can only participate in a maximum of six (6) I Choose Wellness activities per month. Accordingly, the maximum number of points that can be awarded to passport holders for participating in these activities is 570 points.

The maximum number of points that can be awarded for undergoing nutrition counseling is one hundred forty (140) per month.

If I?m an existing Nestlé Club member, how do I earn extra points?

If you are an existing NESTLÉ Club member, you will be rewarded with Nestlé Club (NC) Points upon redemption and validation. Click here to see your NC bonus points. (Link to the table)

Are there any bonus points?

You can double your points at the end of each month if you are e able to comply with the minimum recommended activities of the month.

Members who attend the Wellness Expo on July 4-5 2009 will receive 100 bonus points.

How long is the ICW program?

The passport program will run from April 15, 2009 to December 15, 2009 only.

What happens in case I lose my Wellness Passport or it gets damaged?

For a small fee, a lost or damaged Wellness Passport will be replaced upon submission of the following documents:

  1. Affidavit of loss
  2. Affidavit of at least two (2) disinterested persons attesting to your identity
  3. Certification of the barangay captain that you really are a resident
  4. Other identification papers or documents like office/school ID, SSS, GSIS Number, Tax Identification Number Card, Driver?s License, passport, etc.

The lost or damaged Wellness Passport Number will be deactivated permanently, but points earned as reflected in 1ISA system will be assumed in the replacement Wellness Passport.

Redemption of rewards using a deactivated passport number will not be allowed.

How do I check my points?

To know your available regular or Nestlé Club points, you can check the Monthly Points Tracker of your Passport. You may also do the following:

What are the reward items?

There are two types of rewards.

Note that all rewards will be given on a first come, first served basis until supplies last. Actual reward items may vary from the photographs above. Promotion runs until Dec. 15, 2009 only.

When can I redeem an item?

You can redeem the reward of your choice once you have accumulated enough points. Check the back of your Passport for the list of reward items you can redeem together with the corresponding points.

Terms and Conditions of the Redemption

  1. Redemption of a reward item is subject to all of the following conditions:
    1. Accumulation of sufficient Reward or Fast Track points;
    2. Possession of all the qualifications and none of the disqualifications set forth in these Mechanics;
    3. Proper verification of the identity of the passport holder and the validation of the points earned; and
    4. Approval by NPI/Agency of the redemption.
  2. If after delivery the reward item is discovered to be defective or becomes damaged through no fault or negligence of the passport holder, he/she can request for a replacement of the same reward item within forty eight (48) hours from delivery by calling the Wellness Hotline at 689100. To claim the replacement, the passport holder or his/her duly authorized representative must surrender the defective or damaged reward item in its original manufacturer?s packaging together with any and all instructions and warranty certificates to 1ISA Office at 1 CH King Compound, CJ Caparas Street, Barangay Ugong, Pasig City. Within thirty (30) calendar days from receipt of the said item, NPI/Agency shall notify the passport holder, through telephone call and/or email that a new reward item shall be delivered to him/her, if the reward item is still available, within thirty (30) calendar days from the notification. If unavailable, a reward item of comparable value and nature shall be delivered instead.

    Despite the failure of the passport holder to request for a replacement of the reward item within forty eight (48) hours from delivery, the supplier?s warranty shall remain valid and the passport holder may make a claim against said supplier on the basis of said warranty.

    In any case, after delivery, NPI/Agency shall not be obliged to replace any reward item that has been lost or stolen due to the fault or negligence of the passport holder.

  3. The description of the reward items are based solely on information provided by third party suppliers. Photographs of reward items appearing in the Rewards Booklet may slightly differ from the actual items. In some cases, certain reward items may be covered by a manufacturer?s warranty and the passport holder should direct his/her inquiries to the manufacturer in accordance with the warranty information.
  4. Reward items are subject to availability and the supplier restrictions. Reward items shall be awarded on a first-come, first-served basis and while supplies last.
  5. A passport holder may claim a reward item twice, provided he/she has sufficient points to entitle him/her to redeem both reward items.
  6. The points accumulated herein shall remain valid and effective during the Program run and shall not apply to any extension/s of the Program, if any.

Other Terms and Conditions

  1. All points earned and reward items under this Program are neither transferable nor convertible to cash.
  2. A passport holder is not allowed to pool his/her points with other passport holders to entitle him/her to redeem a reward item.
  3. All reward items shall be awarded tax-free..

After I submit the redemption form, how do I get my reward?

Upon verification and validation, the reward item/s redeemed will be sent via courier service to the address indicated in the form no later than 30 calendar days from redemption.

During delivery, you must present your Passport and at least two valid ID?s with at least one containing your photograph.
